how add condition only for change request if we put query on 'AssociateCIToTask' script include

Hi all,

I wanted add a query on Affected CI related list on change request when users click on Add button on it, but we can see Affected Ci related list also added for incident form. I want to works this query only for change request affected ci related list. anyone please support?
