Analyzing form question data form a catalog record producer

One of our CSM case type includes a catalog record producer with an assessment form used by internal agents fielding customer calls by phone and external end users via CSM portal.  The assessment form is a collection of decision trees to determine if and users request can be immediately or requires additional higher level of care from our expert team.  We want to analyze the data gathered in the assessment form to see where in the question flow users are triggering an escalation to the experts.

 

We want to build a data base view joining the assessment form (record generator) data to the case data, but we can not seem to find where we can access the data captured in the record locator.

 

Can someone point us in the right direction
