Cancel button on Incident Form

Hi Team,

 

I need "Cancel" button on the incident form with below details. 

 

1. Cancel Reason with choices: Duplicate, Open In Error

2. Cancel Reason: String type

1. Once click the submit button, need to set values in "Resolution Code" and Resolution Notes" mentioned in the "Cancel Code'" and "Cancel Reason".

2. Set the state is cancelled

 

Please provide the solution.
