Create a star rating feedback on portal having filled stars

Hi Everyone,

 

I have to create a survey stating 6 questions where 5 questions are having star ratings and one question is description box. I have got it created on the widget and set it on a page but it is not getting saved in the assessment_results table in servicenow. I am attaching the view of my feedback survey form and the widget server script, please help me out if that is possible in ServiceNow or not.

ServiceNow does not support directly saving survey responses into "asmt_assessment_instance" from a custom widget.

To capture star ratings and comments and have them visible in Survey Responses, the correct approach is to use the OOB Survey Designer.
Custom widgets can display data, but survey results must be submitted through the survey engine or its APIs.
