How to find out the request submitted is from which Service Portal

In my Client Instance there are many Service Portal. There is requirement, specific to Portals...

 

If I submit a request from Service Portal (let say portal prefix - pss ) . can we able to find out on the request Record, this record is submitted from this specific Portal..

 

Is there way to find out the request submitted from which Service Portal?

 

Please Help me  finding this..

sp_log table can be used to identify the records created from Portal. Please note that the table also keeps record of views and other activities in portal.
