Maybe a bit late, but in case it is helpful: Did you check the case digest configuration? For the preview button to properly work, the post case review record needs to have a valid record in the "case digest condiguration" field.
The case digest configurations can be created in Case Digest > Administration > Configuration. You will also need to create a Document template.
