ACL on system table not allowing visibility on create

Due to business requirements, we are trying to extend security access on a system table.  To do this, we are creating a separate role that will be used for a limited number of people.  We require this role to have access to create, write, and read permissions, and have set up our ACLs specifically with this role.  While Read and Write are working, the Create permission is not, when we attempt to press the New button on the list screen for this table, we get a blank page as if no fields are visible (I see the Submit button, but that's it).

Is there something regarding the ACLs that I'm missing?  I have done the [form] and [form].* permissions for both create and write so would like to know if I'm missing something.

As you are able to see new button then it is not a create ACL problem. You need to check other Read* and field level ACLs.

Able to see New button.  When I press the New button I get a blank screen with the Submit button.

please go to ui action "New" related to the table you are working on and check the configurations there. 

I see the new button and there is nothing there regarding conditions.  I press the new button, and I get a blank screen with only the "Submit" button.
