Add checkbox field on Incident form

Hello all,

I am trying to add a checkbox field on my incident form, but when I configure the form layout and try to create a new field the checkbox type is not available.

Is adding a checkbox variable even possible on an incident form?

I actually have the same issue. I already have the check box on the call form "Caller not found". But I wanted that when they mark a check on it, they have to fill out the user information manually. How will I do that?

I have requirement to add checkboxes under a mandatory question for a form. How can I add the question along with checkboxes and make it mandatory?
