Add incident details into Surveys

How can I insert task details into surveys? I want to display the number, category, subcategory, and description. 

I want to show the details under the header, before the questions.

I think we can't add the field in the survey, as there is no way to do do walk to a specific table. Survey trigger based on tables but we can't use that table value in survey.

 is your survey based on OOTB table asmt_metric_instance? Or do you utilize a custom table for the survey?

Survey is based on the OOTB asmt_metric_instance table.

Sorry, I think I got the table wrong - isn't it: asmt_assessment_instance? If yes, then you can dot-walk to task.
