Adding ServiceNow Survey Link inside Incident Resolved Email Notification

I am having trouble adding the ServiceNow survey to the Incident Resolved email notification body. I have already created the Assessment Metric Types, added the Trigger Conditions, and added the Metric Categories. I'm having trouble adding the survey to the notification because the assessment instance is not being created and linking to the incident.
In the image, you can see the survey on the header. I want to do something like that in the body

Please try creating the notification on the Assessment Instance table and provide the notification trigger as below conditions(Attached in the screenshot) so that Assessment instance number will be available.
