addition button on the form

hello fellows , I did an app scope and after scope, I realized I have an addition submit button on the form which I did not add, does not show up on the original app

went to the UI action on both forms and both have the button as active but strangely it is not visible on the original form, I would like to make remove/hide one of the buttons but not sure how to mimic the original look on the original form 

Advice please

@June Anderson Please check the condition for both the ui actions. BTW there is one ui action on Global table that will be visible here automatically. We dont need any extra ui action.

the second screenshot is the one I want on the form, both are on global scope and I did and insert and stay and then changed the table name to my new scope and then marked as inactive and its till showing

@June Anderson Please delete all the buttons you created. As submit button is not required to be added to the form. ServiceNow provides it.

So please check the UI Actions created by you and delete them.

And then please post your exact requirement.
