After modification of Questions in OOB Functionality of the risk assessment unable to find Que

Hi,
1.After Modification of the OOB Questions on the Risk Assessment survey , I am able to see those questions in Survey but not able to see those questions on the Risk assessment UI Action.
2. When I add sections on the Risk assessment form Survey unable to see those section on "Risk Assessment" form UI action.

Can anyone please suggest me how to solve those issues.
