Attachment type variable is not responding while clicking on it

I have an Attachment type variable on record producer, when I try to click on upload in service portal, it is NOT responding sometimes. sometimes it works fine.

Please check below screenshot, after clicking on upload it will NOT show anything and If I check console I could see the following errors. Please let me know if anyone of you experiences this issue.

Thanks for the reply. Actually I'm facing issue to add attachment. after clicking on upload button, its remains same not responding.
