Best approach to streamline BCM BIA Smart Assessment form (chart/matrix style view?)

Hello

We have a requirement to improve the user experience of a BCM BIA Smart Assessment form. The current assessment contains multiple questions and the business would like a more streamlined presentation of the information.

We are interested in understanding what options are available to display the assessment in a format such as:

• Matrix-style layout
• Chart-like view
• Grid-based questionnaire
• Any alternative layout that allows users to view and complete information more efficiently
  

Any recommendations, examples, or lessons learned would be greatly appreciated.

What I can recall, we got a requirement to change the looks of a field of BIA Assessment. That we achieved using UI builder.
