CAB Required Check Box and CAB date is not longer populating

CAB Required Check Box and CAB date is not longer populating, See attached. It was working up to two weeks ago and now it won't populate.

How can I recreate this in Personal Instance?  Do you once have a client script/business rule that would set it?  It doesn't seem to set by default on personal instance.  The CAB date field is also not read-only.

It is on an OOB instance at my job. It was automatically set when they turned on Change Module
