Can we submit multiple access requests using one form at once? All the fields must be the same but only user can select different groups and based on the selections of the groups, multiple requests should get created.

Just add a flow to it to handle all entries as separate tasks.

There is no code needed. Just create a flow to handle the request and use the 'create record' actions to create the requested items.
