Change Approval Policy

Hello experts,

I have configured a change approval policy, but despite meeting the decision conditions, change tickets are not going for approval. I have attached screenshots of both my change approval policy and change approval decision. Please guide me. Thank you.

Is it new definition or old? If new, then you need to add in Flow as well.
