This is the "CAB Workbench" which under the hood is just another portal type. There are a few ways to do this but the easiest is with the "Portal Editor"

1. Nav to Service Portal -> Service Portal Configuration
2. In the new window that opens, click "portals" from the top
3. In the drop down change "Service Portal" to "CAB Workbench"
4. Now click "branding editor" from the top
5. Here you can change the logo and theme colors

By CAB form are you referring to the process flow formatter across the top of the form on a change request record?
