Comment functionality in every field like we have in MS word

In workspace, I want to implement the comment functionality in every field like we have in MS word.

please suggest how we can achieve this?

No, we don't have this functionality in ServiceNow OOTB. Not sure what is your use case, but all changes captured in activities or you can use the task management.
