Comment or Activity stream field in approval widget for approver

Team,

I am looking to include a comment or activity stream field In approval widget, so that approver while approving the request can input his/her feedback which the requester can see and action upon.

Currently approval record only display request fields and approve ,reject button but no comments or equivalent field. if any one have implemented such a requirement or have idea ,please let me know .

Any sort of help will be highly appreciated.

Hi Nitin,

I have added a basic comment field to the approval info widget where you can ask approver to add his comments which will be added to the journal field(comment) on the approval record itself. 

Please let me know if you need some help around it.

Could you please copy and paste the code here in the community because we have added other customisations as well to the widget and moving the xml will erase other customisations

I've the same requirement. Also cannot see the option for the attachment on opening the record.

Can you please share your steps on how to create the comments box in the approval page in the service portal?
