Conditional form selection

Hi all,

To reduce a number of visible forms in use (currently 45), I have created a form which initially contains 2 mandatory fields: services and location.

Based on the 'services' field value, I would like to display a different set of fields (map it to different forms).

How should I go about it? Any advice would be very much appreciated.
