Create a form in service portal

When I navigate to service portal and I click on "Request Something" > "How can we help", I would like to create a template so that when I click on it, it displays a form  that will give me ability to report issues directly 

 

Form will just a basic form that should contain

 

  • Requestor: Current user
  • Email
  • Phone
  • Impacted Environment (can select multiple)

           1.Test

           2.Preprod

           3.Prod

  • Category

          1. Inquiry

          2. Network

          3. Database

          4. Account

Hello June,

 

You can create a UI Page or Widget, You can add the routing for that page from the button.

 

Use HTML,CSS,Angular, Client Script, Server Script for creating the Page.
