Create Custom Survey Report

I'm trying to generate a report for my survey responses but want to switch the row and columns.

Here's what my report looks like now with each survey showing multiple rows for every caller:

Here's what I would like my report to look like:

Any ideas?

I have watched this demo twice now and followed along the steps in the PDF.

But still cannot get this to work.

When I join any question, I don't get any results.

If I look at the personalize list choices, the question string values are just not there which tells me the join isn't working.

What can I check?

I have watched this demo twice and followe along with PDF slided, but still cannot get my database view to work.

When I do the second join with any question, I don't get any results.

When I look at "personalize list"  I can't select any question 1 string values.  This choice is just not there like it shows in the demo.

What can I check?
