Creating a field to identify VIP users

I need to create a field to categorize 4 grades of service for users in the user table as a choice list. 

for example 1.Top 2.High 3. Medium. 

Outside of creating this field through form design do i need to have these options mapped to a choices table?

what is a possible solution to approach this task

Hi,

Creating this through Form Designer will also create the choices in sys_choice table.
