Hi @Prasad49
In assessment question stage, the "Source table" field should be mapped to your custom table. I think you selected "assessment metric type".
Also, update "Due date" in stage 1 when you are creating assessment instance. Please correct these two steps and try.

In your Create Assessment instance record flow action, I believe you'll need to input the table name (e.g. "sn_hr_core_case") into the field Trigger table. It looks like you're already putting the sys_id into Trigger ID (rather than a reference field), which is good. Here is a similar post where that was the solution.

after adding the trigger table, trigger ID is reflecting but the questionnaire is not reflecting

I'm not super familiar with assessments. Are you certain that the users have access to see all of the questions? I'd check with the Debug Security Rules module and see if any ACLs are failing for your test user.
