Custom UI Action not showing for adhoc tasking – where to dig?

I enabled adhoc tasking and added a custom UI Action, but it doesn’t appear on the task form. I double-checked roles and conditions, and the script is clean.

Is there another place I should check when a UI Action seems invisible even though it should be active?

is it that you see the button (condition is ok) but it doesn’t do what it should (code is wrong), is that so?
