Data base view for Survey veiw responses in Row

Hello Team, 

 

I have created Database view to view responses in a row, but I'm not getting question in column header.  If you see below screenshot I can see the Value but it's under "String Value" header, but I need question to be displayed instead of "String Value" , Values are correct.

I don't see questions in the available option :
