Date Field on form does not display date picker icon on service portal

Date Field on form does not display date picker icon, hence i don't see the calendar icon & unable to select a date. Please help to fix this?

Appointment is a date/time field without date picker icon on service portal

Is there any script changing the CSS of the portal? Blocking the element from rendering.

This can be either in the cat item itself as a catalog client script (with dom access) or as CSS config on the portal page itself. This can be included in theme used for the portal

i think it might be happening due to css , i'm checking the css codes.

I created a custom page/widget maybe the css is restricting the icon to be visible 😕

It is happening on one particular catalog item for one particular user.
