Direct link to Form widget on service portal homepage

Is there a template I can use to add a form on the service portal homepage that is directly linked to a catalog item? I'm imagining I have to clone an existing an existing widget and apply the direct link to a catalog item so it can populate the form. 

I want this widget form to be static so users can fill out the form for only one catalog item.

Hi @kevin_munguia ,

You can use Icon link widget. When opened in page designer you will get an option to add the URL where u can paste the catalog item URL which when clicked will directly open the catalog item form for the user.

Attaching some screenshot for your reference. When u click on the pencil icon in page designer you will get the 2nd pic display. select type as URL & provide your catalog item URL.
