doc task closed but document not attaching to parent HR case

We have created employment letter automation, where ticket is created and doc task is automatically assigned for verification , once signature done, doc task closed ,but document is not being added to HR case.

Please help here to resolve the issue.

I can see signature in sys_attachment table.

Check you document template state whether it is published.
