Document template not Attach on HR case after signature done

Hi All,

After a document task is completed, ideally the document is generated on attachment table... But for them its not getting generated. On the sys_attachment on the signature png is visible. why the document isn't getting generated?

And document template is not attached on case once provide signature and document task is closed complete.
