E-Signature with custom task table

Hi All,
So I am creating a scoped application. Within this application I have created a custom table that extends 'Task'. Tasks are going to be automatically created and assign a document to a user to sign using e-signature in the 'To-Dos' section of the Employee Service Center.
I have created an E-Signature template that looks like this:
My task looks like this:
But, when I go to the 'To-Dos' section of the employee service portal as the user, as if I were going to sign the document, it shows up like this:
The right hand side should have the document displayed with a signature box underneath it. I have spent many hours digging around and have not be able to figure out how to get this feature working correctly!
Has anyone ever experienced a similar issue, or can provide any insight as to what is going on here?

Im checking the E-Signatures part in the on Demand HR Fundamentals course, but not seeing anything useful there.
From my own POV the form looks incomplete in the widget or the view that widget calls from that table looks to be missing some fields.
