embed an image into a form

Hi Everyone,

I'm trying to put an image into this form.   The users don't need to interact with it in any way it's purely as a visual reference only but I want it to be available every time the users go to submit a new form. Could someone advise an easy way to do this?

We're on Jakarta.

I've been searching the community but not having much luck!

Hi Leslie,

Do you have an after image of how the configuration looked once the image was added? Also, would you be willing to share the UI macro code that you used? I'm looking to do something similar where we show an image inline on the form.
