Enable actionable knowledge feedback Properties but not seeing the pop up box on the portal

Enable actionable knowledge feedback properties in knowledge > admin > properties , but when selects "not helpful" or under 3 stars, not seeing the pop up box on the portal, screen goes gray but nothing pops up for user to enter any feedback, and open F12 for Chrome, we can see error message "Uncaught TypeError: Cannot redefine property: ux_globals" in Console.

 

Any help or recommendation? Thanks in advance.
