Facing issue after submitting the record producer form,in service portal

I have a record producer as mentioned in the screen shot as below, but after submitting this form a different form in opening (redirecting ) in service portal as mentioned as below screen shot. 

 

but we need it to redirect to a black form in portal, can please help how to do it?
