Fetch text entered in GlideModalBox

Once manager reject the assessmnet he would get the GlideModalBox to submit the reason why it is rejected. He will fill the details and submit.

i want to fetch this details entered by manager so that i can send notification to respondent with reject reason.How to achieve this.
