Flow designer action failed

Hi Everyone,

I am having flow designer which attached to Record producer. Once record producer submitted, record will be getting created in Table 'A'. Once record inserted approval also will be triggered at the same time and approvers are member specific group.
After the approval, new record will be created in table 'B' through flow action.

Now issue is that,  flow action has failed and got error as "

The requested flow operation was prohibited by security rules." And record wasn't created in table 'B' as expected even approval provided. Please find the screenshot below.

I have checked on below things to find root cause.
1. Have verified the 'approver' who approved that particular record having required role. He has all role as he is member of approver group.
2. Have verified if the same user had similar issue before and after this issue. I don't find any issues as before and after he had given approval and record were record created successfully without any issues.

 I am not able to replicate the same issue again. I need to find the root cause to avoid this issue in the future.

Could anyone please help here what could be cause for this issue?

Hi @melwintnb,

Just by looking at the error message, it's it may be related to ACL/role.

But I highly recommend you to find a way to replicate the issue again.

Otherwise, there is no way for you to validate whether the fix actually did its job.

Once you can replicate the issue, please provide the steps here and we can assist you further.

Thanks for you reply.!
I couldn't reproduce the same error. But I am getting another issue as "Attempted to add black listed role: [2f66db67531033005ac4ddeeff7b123a]".
Getting this error and flow not moving further. What is the meaning of this error? Could you please help me here?
