Flow designer evaluating to false

Requirement:

Action	Sc_req_item state	Sc_req_item stage
Requested for submit a request	Open	Fulfillment
Assigned to click on Assign to me	Work in Progress	Fulfillment
Assigned change status to Pending	Pending	Delivery
Requested for click change the status to work in progress	Work In progress	Delivery
Assigned to click on close complete or incomplete	Close complete/Close Incomplete	Completed

Hi,

Could you please explain what the issue is?

Some pointers.
You should not need to create a Requested Item records as shown as step 1 in your flow.
To work with stages, check the docs on how to do that.

As per below image I need to create workflow.
When the user submits request the state of the record should be open state so, I added create record

Yes, I can see that from the image.
So the assigned to has a value then, what is it, and where does it get set ?
