Flow Designer set error state not showing

Hi,

I can no longer see the option to set the flow designer action state to error, normally it is right next to the add a stage button but not anymore.

The flow already contains an error state , but I cant set any more.

Please can someone advise how I can add the above.

Few things to check

1) When was the last time it worked fine

As per Paris it has the documentation

It is working, as I can see the option to set the error for one of the actions, but not any other action?

for which action you can see and for which you cannot?

There is one currently set for the below, after the if statement

I can only see the option to set error where the red line is, after update record and before end, I  cant see the error option anywhere else in the form, even though there are similar actions elsewhere. 

Even the action where it had error state previously, I can no longer add it there.
