for each loop is not iterating exactly

i have list collector, based on values choosen those many incidents records will create but here records are creating those many no issue , but when i see the flow execution it's iterating only one time.

in this i have choosen 8 users so in log i got 8 users names  but for lopp executing only one time

any one help me how to slove my issue.

Can you expand step 2 Look Up records action and share the output ?

Also there was a known issue wherein for each loop was working fine but not showing up in the execution but you can see it getting executed from logs. Check if your issue is similar,
