Hello All, I need assistance with troubles with Duplicate emails to assignment group

I have been having issues with duplicate email notification to Assignment group Once request is submitted from the portal. it is triggering from the email notification I created in RITM table, I have checked the flow, business rules, scripts but to no avail.

 

I even deactivated the email notification to create another one using just "active is true" AND "Assignment group changes to the group" AND "assigned to is empty" - yet the email notification still comes out as duplicate.

what's the trigger condition for that?

share screenshots of that notification

may be some business rule has current.insert() or current.update() which is causing the email again to trigger.

I share same email notification with some of my catalog items in Team 03, funny thing is that It was functioning right before but after cloning and upgrade it started duplicating emails, but in test instance triggers just one email even though test and dev has same conditions

 

Below is a new notification I created deactivating the first one above just to test it out yet it still comes out as duplicates.
