Hiding fields based on answer for fulfiller Incident form

Hello, I am looking for some guidance on the following.  We have some questions that are asked on the Service Portal incident form.  When they carry over to the fulfiller side based on the answer, we would like the option to hide some of the fields.  Below is an example of one scenario.

I have created UI polices and actions on the portal side to drive what questions are displayed based on choice but i'm not familiar on how this is done on the fulfiller incident form. 

 

Any assistance to point me to the right steps would be much appreciated.

You'll create a UI Policy on the resolver pages instead of a Catalog UI policy, it's similar but tricky.

Simplest way is to create UI Policy on the incident form.
