How to add a PDF form on the Catalog item on Service Portal

Need to add a PDF form on the Catalog item on Service Portal.

As when the user select a option from drop down list a PDF form link should get populate in the Top as mentioned in the screen shot. Once User click on the form link then PDF form should appear. After filling the form then only User must be able to submit the request.
