I create Change Request and click Request Approval as picture below.

How to add attachments from Change Request to Approval Request email?

there is a field in notification "Include Attachment" kindly checked that. then attachment will get attached in the notification.

I tried your solution, but it still has no attachment.

Set Include Attachment to checked.

Create Change Request and click Request Approval.

Receive Approval Request email, but it still has no attachment.
