Out of the box the approval email doesn't have RITM attachments

so what did you start with?

2 ways

1) include link to attachment in the approval email

OR

2) copy the file from RITM to Approval record when Approval is inserted and then use "Include Attachment" checkbox on OOTB Approval email notification
