How to add column label for simple list in Service Portal

Hi Everyone,

Could you please help me on the below requirement. 

We have created a custom simple list in which it will show Cases related info, my requirement is i need to show column labels like Number, State like that as shown below.

If anybody knows how to do this kindly help me.

Since you created it custom, please share what you created, so we can see how you can add the column names. Otherwise it would be guessing.
