How to connect a Declarative action button to a modal in Playbook

Hello everyone,
How can I set a declarative action that was created for playbook to be able to show a modal upon clicking? I tried searching articles in the internet and what's common in them is to create an UX Add-on Event Mappings which is not visible in the declarative action for playbook, only Playbook Experience Mapping related list. Is there a workaround for this? I appreciate any of your help, thank you very much.

Hello @Ankur Bawiskar, thank you for your response.
The declarative action button is already visible and I set it to be displayed in User Form and Questionnaire, my problem is how to connect the custom modal that I have created to the declarative action button. For now, when I click the button it does not do anything even if i already cleared the cache.

There is no OOTB button that functions the same way as I want to, that can be used as reference.
