How to create a module same as "Create New" [Change>Create New] under a custom application.

Hi Everyone, 
I want to create a module like 'Create New' under 'Change' Application[ "Change> Create New"] in my custom application where I can add some cards like the below images. If the user clicks on these cards they will be redirected to some table. 
In simple terms, I want to create an interceptor with some cards that are used to redirect to some table.
