how to create pop-up for ui action

Hi everyone,

i need to create one button Request Approval on change   form so that it will prompts with a pop-up and allows enter a message that will be added in the email that goes out to the client lead requesting approval.

So could you please give some idea how to do this.

I have doubt

1.           It looks like the request approval button is just placing text in the additional comments field. Two problems with this, this is not part of the notification that goes out to the approvers. They get a copy of the change plan from the planning tab.   By adding the text to the additional comments field it is sending a separate email to the requester and watch/work notes list users but not the approver. We need this text to be added to the existing notification that goes out called "Approval Request". Second, pressing the button doesn't save the form or start the approval process.   Does pressing this button have any additional functionality besides adding text to the additional comments field?

2.           The request approval button only is available on the form view when the change is in the "not yet requested" approval state.   Can we make this button be available even after the state changes to "lead approval requested" but change the text on the button to say "Restart Approval Process" and have it actually restart the approval process when pressed?

Can you please give some idea
