How to create Scheduled Requests based on certain data input dynamically

Hi ,
We have a requirement to schedule Request ticket creation based on a certain frequency and assign to certain assignment groups and assignees .
To schedule request creations - Monthly /weekly/ biweekly/one off to mentioned assignment group by the user dynamically . Input screen attached. Also how to modify,maintain amd report on these schedules at any point.

You can do via Flow Designer.

Schedule
Action
create Record
and Update the fields in same
If you want same as attached, then you need to create a full length catalog item and build a logic via flow deisgner.
